the completed application form shall be accompanied by the following documents:
1. copy of university diploma (MUDr.) - Officially certified4
2. When Study was started v Former Czechoslovakia before 1 January 1993 and you worked on the territory of the Slovak Republic for a minimum period of three years last five years before the application, submit also:
(a) confirmation from employer5 on professional practice in health care doctor in the territory of the Slovak Republic (original or certified4 copy); model practice certificates for download here in format pdfor rtf;
(b) Officially certified4 a copy of the document confirming the registration of the Slovak Medical chambers
or
if you have periods of at least three years in the last five years have worked on the territory of the Czech Republic before the application is submitted; the competent authority of the Czech Republic (MZ ČR) for the issue of the document.
3. not verified a copy of the identity document (both sides) E.g. ID card, or copy passport - if you are an EU citizen,
or
a copy of the passport - if you are not an EU citizen
If a copy is submitted a passport if the name on it is not identical to that on the educational document, e.g. when married, submit with him another document identical to the name on evidence of education. That information shall apply to all applicants.
