Act No. 540/2021 Coll. on the categorisation of institutional health care Articles 12 to 14 regulate a number of types of applications:
- Request for conditional inclusion of a hospital in the network, request for conditional increase in the level of the hospital included in the network, request for conditional provision of a supplementary programme,
- Request for modification of data on the place of operation, the number of beds and the distribution of compulsory programmes between the main hospital and the partner hospital,
- Request for a reduction in the level of the hospital included in the network, request for the abolition of the supplementary programme in the networked hospital and request for the removal of the hospital from the network.
Hospital operator sends the request in writing until 30 April of the calendar year must contain all necessary particulars and annexes as defined by law. In the absence of relevant documents, the Ministry of Health immediately invites the party to complete them within 10 days. If the Ministry of Health has not been corrected, he will stop the proceedings.
The party to the proceedings shall at the same time support the application proof of payment of the administrative fee by transfer to the bank account of the Ministry of Health. The time limit for receipt of proof of payment shall be 10 working days from the date of application..
