The application requirements include the "selection procedure for authorisation - conditions" which can be requested in writing or via the Internet at the e-mail address veduca.uradu@health.gov.sk. The application for a permit to operate an ambulance of a ground emergency medical service shall indicate the location of the ground emergency medical service station for which the tenderer applies for an authorisation to operate.

The application, together with the relevant documents, shall be submitted in writing, in the Slovak language, in two copies, inserted into one envelope for the issue of a permit to operate a ground ambulance for emergency medical services. Applications shall be certified by the signature and stamp of the applicant for authorisation. The application envelope must be secured against possible premature opening and contain a warning "Selection procedure for the authorisation - NON-OTVARAT', with the address of the sender, number and name of the organisation, for the natural person name, surname, title.
Envelopes with applications which arrive after the deadline shall not be included in the tender. Requests can be delivered by registered mail. Personal service at the address of the notifier is inadmissible, requests thus received shall be excluded from the selection procedure.
The tenderer shall not reimburse the tenderer for any costs associated with participating in this invitation to tender.
The tenderer reserves the right to:
1. Cancel and close the announced selection procedure without determining the best participant.
2. Amend the terms of the selection procedure by making the amendment public in the manner that this tender is launched.
3. In the case of non-compliant applications, reject applications and terminate the selection procedure as unsuccessful, including for individual premises.
By submitting an application for authorisation, the tenderer shall give its consent to the terms of the invitation to tender.
