The detailed requirements and criteria for applying for an authorisation to operate an ambulance of the emergency medical service include the conditions for the selection procedure for the issue of a permit for the operation of emergency medical services', which can be collected at the secretariat of the Head of Office of the Ministry of Health of the Slovak Republic on working days from 10.00 to 14.00. The selection procedure for the issue of a permit to operate an ambulance of emergency medical services shall be declared for each declared establishment of the ground emergency medical service separately, i.e. applications shall be submitted separately for each declared station seat.
Adoption of tender conditions: from 17 October 2013
Period for lodging applications: 16 December 2013
The date of filing on the postmark is decisive.
The selection panel shall invite in writing participants who fulfil the conditions of the selection procedure to tender at least 5 days before the start of the selection procedure.
Applications for an invitation to tender for a permit to operate an ambulance of emergency medical services shall be submitted in writing in the Slovak language in two copies, inserted in one envelope. Applications shall be certified by the signature and stamp of the tenderer. The cover with the application must be secured against a possible premature opening; it must include the warning "Selection procedure for the issue of an emergency medical service ambulance - NEOTVARAT" with the address of the consignor, the identification number and the name of the organisation, name, surname and title for the natural person. The envelope shall also include the location of the ZZS-station and the type of ZZS-station (RLP, RZP). Envelopes with applications which arrive after the deadline shall not be included in the tender. Envelopes with applications that do not contain all the required data will be excluded from the tender procedure. Applications shall be delivered by registered mail. Personal service at the address of the notifier is inadmissible, requests thus received shall be excluded from the selection procedure.
The tenderer shall not reimburse the tenderers for any costs associated with participating in the selection procedure.
The issuer reserves the right to:
- The notified selection procedure shall be cancelled and terminated without the designation of the party best meeting the selection criteria for each declared establishment of the ground-based emergency service separately.
- The amendment to the invitation to tender shall be notified by the manner in which the invitation to tender is launched.
- In the case of non-compliant applications, reject all applications and terminate the selection procedure as unsuccessful, for each declared seat separately.
By submitting an application, the tenderer shall give his consent to the terms of the invitation to tender.
