- written consent of the data subject to the processing of personal data data for the purpose of choice under Law No 428/2002 Z.z. on personal protection data as amended.

Applications bearing the tender reference number and the documents required shall be
to be sent to the Ministry of Health of the Slovak Republic, Personal Office,
Limbova 2, P.O. BOX 52, 837 52 Bratislava, no later than 08 October 2012.
