The Chairman of the Government of the Slovak Republic, responsible for the management of the Ministry of Health Peter Pellegrini in accordance with its legal scope resulting from § 22 (2) of Act No 581/2004 Coll. on Health Insurance, Health Care Supervision and amending certain laws as amended (hereinafter referred to as "the Act') is interested to propose to the Government of the Slovak Republic a candidate for the Chairman of the Office for Health Care Supervision. In order to fill the office president in a transparent and open manner with the possibility to apply for this function to all candidates fulfilling the legal conditions The Ministry of Health of the Slovak Republic invites those interested in the function of president of the office fulfilling the statutory conditions so that by the date of January 24, 2020 with the designation "UDZS- do not open' on the cover to the address: Ministry of Health of the Slovak Republic, Office of the Minister of Health of the Slovak Republic, Limbová 2, P.O.BOX 52, 837 52 Bratislava:
- the CV by which the applicant demonstrates compliance with the conditions laid down in Article 22 (2). 4 letter (a), (c) and (e) the Law,
- a copy of the evidence of educational attainment showing compliance with the condition of § 22 (4) (a). (b) the law,
- a self-declaration of integrity;
- a self-declaration of credibility within the meaning of Paragraph 33 (3) (a). (a) the law,
- a self-declaration by the tenderer that the information contained in the CV is true,
- consent to the processing of personal data in connection with the assessment of the person's documents submitted and/or with a view to the disclosure of personal data in connection with this invitation and/or to the provision of personal data to third parties.
When served by post, the date of submission is indicated on the postmark, the date of receipt is indicated on the registration stamp by the Ministry of Health of the Slovak Republic. The Ministry of Health of the Slovak Republic will inform about the further procedure for selecting a person nominated as president of the office in the following period.
