Department of Medicinal Policy of the Ministry of the Slovak Republic in accordance with the application of the international obligations undertaken under the United Nations Conventions (Universal Convention on Narcotic Drugs of 1961, Convention on Psychotropic Substances of 1971, Convention against Illicit Trade in Narcotic Drugs and Psychotropic Substances of 1988) and pursuant to Law No 139/1998 Z.z. on narcotic drugs, psychotropic substances and preparations as amended by Law 260/1999 Z.z. and Law No 13/2004 Z.z., publishes the forms and instructions below.

I. Forms and instructions for forms concerning applications for import and export and transit of narcotic drugs, psychotropic substances and preparations:

II. Forms and instructions for forms concerning the reporting obligations of holders of authorisations to handle narcotic drugs, psychotropic substances and preparations:

MZ SR publishes these forms in order to speed up the processing of the agenda concerning the legal movement of narcotic drugs, psychotropic substances and preparations and submitted to the UN authorities, as well as to unify and clarify the requests submitted, as well as to facilitate the fulfilment of reporting obligations by holders of authorisations to handle narcotic drugs, psychotropic substances and preparations.
