The payment for disclosure shall be determined as the sum of the individual costs. If the material cost of making the information available does not exceed 1,99 €/ 59,95,- CZK, the Ministry of Health will not require the applicant to pay the costs.
The fee for the provision of information, exceeding 1,99 €/ 59,95 ,-Sk, can be paid at the cashier of the Ministry of Health of the Slovak Republic, Limbová 2, 837 52 Bratislava in working time, or by post order or by cashless transfer to the O. O. MZ SR: 70001543/8180, variable symbol: VS 211.
