By letter dated 23.12.2015, the State Institute for the Control of Medicinal Products informed Ministry of Health, that on 11. 12. 2015 entered into force Decision of 19.11.2015 of the State Institute for the Control of Medicinal Products 4980/2015/680 on the suspension of the special authorisation for treatment with designated substances of category 1 of Annex I to Regulation (EC) No 273/2004 drug precursors, reg. No.: ZAO-UL-64/2011 dated 14.4.2011 for 6 months, i.e. until 10.6.2016 for the holder of the medical authorisation care company MEDALIF s.r.o., Jednoradová 997/1, 908 51 Holíč, Location of activity: Public Pharmacy MEDICAL LUCHY, Jednoradová 997/I, No.: 45 690 537.
It follows from this that to the public pharmacy medical device 997/I, 9O8 51 Holíč, ID: 45 690 537 in accordance with Article 3 (3) of Law No 362/2011 on medicinal products and medical devices and amending certain Laws as amended during the suspension period authorisations for the treatment of scheduled substances of category 1 of Annex I Regulation (EC) No 273/2004 on drug precursors, Reg. medicinal products containing drug precursors may not be supplied on 14 April 2011.
