The Ministry of Health of the Slovak Republic pursuant to § 11 and 20 of the Act No. 18/1996 Coll. on Prices, as amended, provides:

Art. I

The measure of the Ministry of Health of the Slovak Republic No 07045/2003-OAP of 30 December 2003 laying down the scope for regulating prices in the field of health (communication No 588/2003 Coll.) is amended as follows:

1. In Annex 2 , Part IX , the following sentence is added at the end:
The maximum price includes all costs of donor testing, blood sampling, blood testing and the costs of processing, preparation and storage of transfusional medicinal products and profit.

2. Annex 4 makes the amendments listed in the Annex to this measure.

Art. II

This measure shall enter into force on 1 April 2004.

Amendments to Annex 4 to Measure 07045-2/2003-OAP

1. In the introductory sentence of Part A, the words "social security' in accordance with the Law of the National Council of the Slovak Republic No 274/1994 Coll. on Social Insurance as amended" and the words "Law of the National Council of the Slovak Republic No 387/1996 Coll. on Employment, as amended" are replaced by the words "Law of the National Council of the Slovak Republic No 461/2003 Coll. on Social Insurance as amended."

2. In Part A, item 1 (a) (a) the amount of

8. In Part A, item 1 (a) (h) the amount of –0,20 This Regulation shall be binding in its entirety and directly applicable in all Member States.

9. In Part A, item 2 (a) (a) the amount of –0,38 This amount shall be replaced by the amount of Þ0,40 Sk

10. In Part A, item 2 (b) reads as follows:
(b) The price of the health performance point listed in Annex 1 to the Code of Treatment for Social Purposes is 0.50 Sk.

11. In part A item 5, point (c) is deleted.

12. In Part A, the title of heading 6 reads: "Save medical service""

13. in Part A, entry 6, the following point (d) is inserted after point (c):
(d) The maximum price per kilometre of driving by a vehicle for the secondary transport of critically ill persons and persons with present failure or immediate risk of failure of any organ system is 15 Sk. The operator of such transport in the field of anesthesiology and intensive medicine in the vehicle for this specially material-technically adapted to the price per kilometre of driving adds a monthly amount per insured person at a maximum amount of 0.10 Sk.
The current point (d) is referred to as point (e).

14. In Part A, item 6 (e) reads as follows:
(e) The performance price under points (a) and (d) does not include costs under heading 1 (a). (c) the first point.

15. In Part A, item 7 (a) (d) the introductory phrase shall read:
The costs of performance referred to in points (a) and (b) shall not include the costs of

  1. primary medical care provided by adult practitioners, practitioners for children and adolescents, adolescents, gynaecologists
  2. dental procedures,
  3. secondary and subsequent healthcare,
  4. joint investigation and treatment components;
  5. medical first aid service,
  6. transport to, from and between health establishments;
  7. emergency medical services,
  8. the performances referred to in Part A, item 10,
  9. blood transfusions prepared pursuant to § 29a of Act No 140/1998 Coll. listed in Annex 2 to Part IX of the measures of the Ministry of Health of the Slovak Republic No 07045/2003-OAP of 30 December 2003,
  10. extremely costly healthcare as the only therapeutic alternative provided to patients in exceptional cases, subject to the prior consent of the reviewer;
  11. medical devices for costly performances as listed in the following table:

16. in Part B, the third point is deleted;
