Vaccination against COVID-19 disease

The registration form for vaccination against COVID-19 disease is available at https://vaccinacia.nzisk.sk/registration

Registration is possible for the following groups:

1) Insurance policyholders in Slovakia

Note: vaccination is also available to a person published in the list of debtors of the health insurance undertaking

2) Persons with registered residence form in the health insurance company in Slovakia

- identifier in the login form: birth number / meaningless identification number
- show up before vaccination: insured person's card (issued by a Slovak health insurance company) with the designation
- the payment for vaccination is made by: the health insurance undertaking concerned

3) Persons without health insurance with full coverage of health care (persons with supplementary protection granted, persons included in the MV SR victim protection programme)

- identifier in the login form: identification document number
- show up before vaccination: Special VZP ID
- the payment for vaccination is made by: VZP / refund Slovak Republic

4) Citizens of the Slovak Republic without public health insurance

- identifier in the login form: Birth number
- show up before vaccination: ID of Slovakia or passport of Slovakia
- the payment for vaccination is made by: VZP / refund Slovak Republic

5) Persons without public health insurance who are students of HEIs in Slovakia and are also in employment relationship with the healthcare provider

- identifier in the login form: Residence permit number / ISIC number / travel document number
- show up before vaccination: confirmation of visit to HEIs and contract of employment with healthcare provider in Slovakia

- the payment for vaccination is made by: VZP / refund Slovak Republic

6) Persons without public health insurance granted (tolerated, registered, temporary, long-term or permanent residence) in the territory of the Slovak Republic

- identifier in the login form: residence permit number or diplomat card number and diplomatic mission staff in Slovakia
- show up before vaccination: Document (s) on residence permit in Slovakia or diplomat card and staff of diplomatic mission in Slovakia
- the payment for vaccination is made by: VZP / refund Slovak Republic

7) Persons without public health insurance who are students of HEIs in Slovakia in full-time study form from another EU and EEA Member State

- identifier in the login form: European Health Insurance Card identifier
- show up before vaccination: confirmation of HE visit in Slovakia and European Health Insurance Card
- the payment for vaccination is made by: health insurance undertaking

8) Persons without public health insurance who are pupils of primary school in Slovakia or secondary school in Slovakia from another EU and EEA Member State

- identifier in the login form: European Health Insurance Card identifier
- show up before vaccination: confirmation of visit to the school in Slovakia and the European Health Insurance Card
- the payment for vaccination is made by: health insurance undertaking

9) Asylum seekers in Slovakia whose applications have not yet been decided

- identifier in the login form: asylum seeker's card number
- show up before vaccination: asylum seeker's card
- the payment for vaccination is made by: VZP / refund Slovak Republic

10) Applicants for temporary refuge in Slovakia, whose applications have not yet been decided

- identifier in the form to sign in: number of the applicant's temporary refuge card

11) Persons without public health insurance residing in the territory of Ukraine, residing in the Slovak Republic

- identifier in the form For registration: Ukrainian identification document number

Invoicing healthcare provider the costs of the groups referred to in points 3 to 6 and 9 to 11 (invoice to the MFF) submit, as an annex to the invoice, a list of persons to whom vaccination has been granted. The list shall be submitted separately for each group. For Group 11 attach a copy of the identification document.
