Priority areas of action
Strategic guidelines to improve the health and development of children and adolescents are based on the implementation of the European Strategy, which highlights seven main priorities:
1. Mother and newborn health
The health of the child is inextricably linked to the mother's health arising from her reproductive health, way of life during pregnancy and attitude towards breastfeeding. Antenatal, perinatal and subsequent healthcare is important for optimal development of the child until adulthood.
2. Nutrition and physical activity
Healthy nutrition is the basis for healthy development of the child. Very important is
ensuring that as many children as possible are breast-feeding as long as possible. Wrong
nutrition leads to health impairment and deterioration of nutrition status. Presents
an increasing problem in almost all European countries. May lead to
obesity in school children and increase the risk of cardiovascular and other
later in life.
Adequate and balanced physical activity is a condition
healthy physical and mental development of children and adolescents. It also supports personal
development is effective in the prevention of physical and mental illness, in particular
chronic non-infectious and cardiovascular diseases. Meaningful digestion
leisure helps to prevent addictions to tobacco, alcohol, narcotics; and
psychotropic agents.
3. Infectious diseases
Acute respiratory diseases, diarrheal diseases and tuberculosis are causes of death, and poor health of children. To a large extent, they may be prevented or treat. However, they still represent important causes of mortality in many countries. Diseases that can be prevented from vaccinations are generally good controlled. Sexually transmitted diseases, diseases from Intravenous administration of narcotic drugs and psychotropic substances associated with the use of olanzapine is not recommended. the transmission of many infectious diseases.
4. Injury and violence
Injury and violence during childhood and adolescence carry a high risk of long-term physical and psychosocial consequences. Traffic injuries and domestic violence The healthy development of children and adolescents is particularly burdensome.
5. Environment
Children's organism is more sensitive to the effects of undesirable factors in life environments such as lack of clean water, poor or poor hygiene, polluted external or internal environment, but also many chemicals from environment. Children from socially disadvantaged people are particularly exposed to this problem population groups.
6. Age
It's the age of experimentation, sometimes even risky behavior. During this stages of development create lifelong habits that can affect health for life. Growth should be actively involved in addressing its problems and participate in protecting your health.
7. Psychosocial development and mental health
The disease is increasing in this area. There are few programmes aimed at preparing for motherhood and parenthood, to support parental skills improving the psychological outlook of the young generation. Suicide rates a depressed conditions are often associated with poor learning outcomes; disorganized family background, tobacco, alcohol, narcotics, and psychotropic substances as well as nutritional disorders. The biggest problem we find u marginalised and disadvantaged groups.
1. MOTHER AND NOVORDAY HEALTH
Care for a pregnant woman, a fetus and a baby after birth belongs to all developed countries among the most important health projects with high social and economic returns. Long-term care for the child so disability is not only financially life-long difficult, but is often causing many families to break up.
1. 1 Background
In the Slovak Republic, since 1996 the project of the perinatal quality has been implemented care in the Slovak Republic- approved by the Government Resolution Slovak Republic No 389 of 4.6.1996 on state family policy in cooperation with WHO, UNICEF and PHARE. In connection with this project a regular joint annual quantitative analysis is provided; and qualitative indicators for the provision of perinatal and neonatal health care (Tab. No 1).
Out of 66 neonatal workplaces in the Slovak Republic, only 23 health institutional healthcare facilities have met the WHO and UNICEF criteria on obtaining a certificate Mother and baby friendly hospital initiative. This the initiative stresses the inseparability of mother-child ties, the importance of quality prenatal and perinatal care not only for children age, but for the whole life. Includes elements of the breastfeeding support programme, support for physiological motherhood and behavioral approach in joint nursing of newborns and mothers. Obligation support mother and baby friendly hospital initiative has been given terms and conditions The UN Committee on the Rights of the Child.
As part of early detection and treatment of newborn diseases in the Slovak Republic conduct ultrasound screening of congenital foetal development defects, morbus detection down and other genetic diseases, in neonates screening for cystic fibrosis determination of immunoreactive trypsin in a dry drop of blood, phenylketonuria, congenital hypothyroidism, congenital adrenal hyperplasia, cataracts, disorders hearing, ultrasound scan of the kidneys and congenital hip luxation (Table No 1).
To compare national health and social policy levels use neonatal mortality as one of the main indicators. Indicator evaluated in all countries of the European Union, evaluated by the World Health Agency organisation setting out care programmes on its basis the youngest population. In evaluating the results achieved in Slovakia The Republic compared to other countries in Europe, it should be noted that lagging behind in terms of infant mortality statistics. Over the last ten years registered by the Slovak Republic twice as high as infant mortality. in the Czech Republic, due to increased infant mortality of social risk groups; associated with lower birth age and lower awareness in the area prevention, sexual and reproductive education and care of the child. From statistical surveillance shows that so much dies during the first week of life children, as from the twentieth eighth day of the first year of life to the 40th year of life.
On adverse trends in neonatal mortality in the Slovak Republic insufficient intra- and post-natal centralisation of high risk fruits and critically ill newborns in specialized workplaces peri-natal centres.
In the care of mother and child and support of community care for mother and the child has not yet been accepted by the Slovak Government programme. Similar programmes were successfully implemented several years ago in The original Member States of the European Union and in 1992 also Czech Republic Republic. Jobs of the doctor's staff of general health care for children and adolescents, assistants and nurses providing nursing care to newborns. Names participated in searching and managing the care of pregnant women and children in social risk groups and marginalised Roma communities. In particular, knowledge of external and internal services has strengthened the efficiency of these types of services. family/parent environment, immediate contact, individual access, increased provision of care information; and prevention of the health of both mother and child.
Today, healthcare is provided by outpatient health care Gynecology and obstetrics and general practitioners Children and adolescents.
1. 2 Objectives
1. Centralisation of women at risk of pregnancy for the provision of specialised health care to the newborn whose prognosis is affected by the right and early intrauterine, intra- and postnatal therapy, but in particular a time factor for transport to a specialised neonatological site; or specialized in neonatal and paediatric surgery to reduce neonatal morbidity and mortality.
2. Improving health protection during pregnancy, preventing possible harm Fetals related to tobacco, alcohol, narcotic drugs and psychotropic substances during pregnancy and increasing awareness and promotion. Prevention of possible damage fetal and neonatal in preventable and intrauterine treatable diseases such as a group of malformations caused by the most common teratogenicity of microbial origin, Syphilis, Toxoplasma gondii, Rubeola, Cytomegalovirus and Herpes simplex Sexually Transmissible Diseases hypoxia, congenital developmental defects and other infections.
3. Safe Motherhood - the goal is to ensure optimal care for pregnancy wife and newborn. This includes efforts to reduce maternal mortality and morbidity, as well as improving the health of newborns by an organisation equivalent to access to healthcare, including family planning, with emphasis to marginalised Roma communities and other disadvantaged groups. Pediatrician and general physician for children and adolescents promote breastfeeding and healthy nutrition newborn. These activities need to be integrated into health policy the State on which development programmes will be drawn up, the provision of services; providing training, including information campaigns aimed at conscious, healthy, human behaviour and elimination of inequality in access to health care by supporting community work in the field Health education.

2. NUTRITION AND PHYSICAL ACTIVITY
Excessive consumption of a diet that does not match the child's age with its composition, movement activity leads to an increase in obesity, especially in school age, but also pre-school. Paediatric nutrition disorders and deficiency movement activity creates a significant prerequisite for the emergence of "civilization" diseases and cardiovascular diseases. Risk factors that accelerate development cardiovascular diseases, they have been active since early childhood and are reflected above all as negative consequences of life administration and wrong way of life.
The emphasis on proper nutrition is from birth by promoting breastfeeding, rational diets and healthy lifestyles in individual development periods the child.
2. 1 Background
On 13 December 2006, the UN approved the UN Convention on the Rights of Persons with disabilities, signed by representatives of the European the Commission and in September 2007 the President of the Slovak Republic, it is certain that In the near future, this document will be developed and guided by Member States the European Union to implement it in national legislation. Due to this Facts are needed in the National Programme of Care and Children and Growing Up 2008-2015 to pay more attention to disability issues children and adolescents. Early diagnosis and early intervention is possible prevent serious consequences of various birth defects, or it is possible achieving greater success in preparing for the future profession, as independent as possible integrated life in society.

3. INFECTION DISEASES
Prevention of infectious (communicable) diseases in Slovakia is implemented based on The Ministry of Health of the Slovak Republic Public Health Office of the Slovak Republic plans, coordinates and determines the scope and vaccination schedules by means of the National Immunisation Programme and the vaccination calendar.
3. 1 Background
Today, the vaccination calendar in Slovakia is progressive with its three-dose a scheme in the first year of inclusion of hexavaccins using a cellular component pertussis, but revaccination in 3rd and 6th year is carried out by obsolete whole cell and highly reactogenic vaccines. Many countries of the world just like first included acellular booster doses in the vaccination calendars pertussis vaccines. The situation needs to be addressed as the child who received the acellular pertussis vaccine should not be revaccinated whole cell. Done, the older the baby, the more negative it responds to vaccination. Unsolved serious reactions after vaccination.
The incidence of pertussis is increasing in the surrounding countries, especially among adolescents and adolescents. adults. This is due to a decrease in the incidence of Bordetella pertussiswhich ensure natural dosage when the disease occurs. Bordetella pertussis endangers unvaccinated newborns (up to 10 weeks).
After compulsory vaccination of infants against infections caused by Haemophilus influenzae type b) invasive pneumococcal infections have occurred the most common cause of death or permanent damage due to infection in the first two years of life. That circumstance was the decisive motive for protection small children in the first 5 years of life with vaccine conjugate vaccination. Invasive pneumococcal infections represent the highest risk for the group of youngest children And old people over 60. Even in EU countries that have vaccinated selected groups The population is mass-introduced whole-area vaccination with pneumococcal conjugate vaccine. Such a form is highly topical to us. They also allow rationalising steps such as reducing the number of doses, thereby harmonising vaccination for pneumococcal use in infants with hexavacin.
Vaccines against Neisseria meningitidis and against rotavirus infections require high payments from parents. Their reduction would be more supportive Parents' willingness to pay for vaccination, which is included in many European countries the weight of the vaccination. Increasing population revaccination reduces frequency population.
Papillomavirus infection can lead to cervical cancer. V Slovakia is diagnosed with more than 500 cancer cases per year The cervix and about 120 women die each year. Development of vaccine against Papillomavirus is a breakthrough in one direction. At proven 80-100% causal association of papillomavirus infection and development of cervical cancer uterine is a vaccine against this infection together with improved screening women the only way to fight this insidious disease.
Areas with higher concentration of marginalised Roma communities occur every year to the occurrence of local hepatitis A epidemics when vaccinated all contacts at the discretion of a regional epidemiologist. Past vaccination of 2-year-old children in these regions was standard. Continuation continuous vaccination may reduce the incidence of epidemics in these regions and to further restrict the circulation of hepatitis A virus in Slovakia.
In the field of oral health, based on the European strategy, the Slovak Republic does not have a comprehensive study in children and adolescents in the age groups of 5 years, 12 years and 15 years old. These data are missing from the WHO database.
Dental caries and periodontal diseases are chronic non-communicable infectious diseases that have a negative impact on health and cause diseases of others organs of the human body, e.g.: infections of the heart, kidneys, but also cause risk of pregnancy associated with premature delivery and low weight newborn.
No less important is the treatment of children's infections. It is generally known that
Slovakia belongs to countries with the highest consumption of antibiotics. It's growing
the problem of antibiotic resistance to many pathogens. A large proportion of childhood infections
primarily respiratory tract is caused by viruses in which
antibiotic treatment effect. Clinically, it is sometimes very difficult to distinguish between viral and
bacterial infection, especially in the clinic of general physician for children and adolescents.
C-reactive protein is one of the positive proteins of the acute phase of inflammation. His
the rise in the body is a sign of bacterial inflammation. Reaching peak in range
24-48 hours. Monitoring of C-reactive protein allows monitoring of intensity
the pathological process and also control the effectiveness of treatment procedures. Right.
disease determination and targeted antibiotic treatment has significant economic significance
and saving the consumption of antibiotics.
3. 2 Objectives
1. Assessment of the current vaccination calendar in the context of the extension of compulsory vaccinations against pneumococcal infections, vaccinations teenage girls against Papillomavirus infection, infant vaccination against pertussis, including in adolescents, acellular pertussis vaccination vaccines (booster dose) and regions with higher marginalised numbers Roma communities on vaccination against hepatitis A.
2. Create suitable conditions for a higher availability of the vaccine, especially in rotavirus vaccines in the form of financial support for children from low income families focusing on marginalised Roma communities and other socially disadvantaged groups population.
3. Reducing dental caries by the most effective method of prevention economically easy compared to treatment alone. At Ministry level SR education for kindergartens and primary schools to renew already in 1998 approved supplement on teaching plans on the importance of oral health using the text recommended by WHO.
4. Educating children in kindergartens and in 1.- 3rd grade of primary schools Health education on oral health, healthy nutrition with reference to healthy food and the importance of oral health for body health.
5. Development of an epidemiological study on oral health in children and adolescents in age groups of 5 years old, 12 years old and 15 years old to meet WHO conditions; re-evaluate these data at 5 year intervals.
6. From infancy to creating awareness of the importance of oral care Health, in particular age-related habits with adequate access to oral hygiene and healthy nutrition. In cooperation with dentists, dental Hygiene gradual introduction in Slovakia to implement local fluoride methods in PS and this primary preventive effect should be assessed within the epidemiological studies.
7. Focus on preventing and reducing the occurrence of parasites and infectious diseases children from marginalised Roma communities.
8. Allow in general clinicians for children and adolescents for infectious infections to C reactive protein to distinguish bacterial a viral inflammation and administration of targeted antibiotic therapy.

4. IMPACTS AND VIOLENCE
Injury is one of the most common causes of death in children in the long term age. Injuries during childhood and adolescence carry a high risk of long-term physical consequences and psychosocial harm. Incidence of injuries and domestic violence is particularly high.
While overall child mortality is decreasing worldwide, deaths related to injuries fall only insignificantly.
In a study by independent expert Paul Sérgio on violence against children, Pinheir, under the authority of the Secretary General of the United Nations, is violence defined as the deliberate use of physical force, whether actual or in form intimidation that results in or is very likely to have result of injury, death, psychological injury, impaired development or feeling alienation. Violence against children involves physical and psychological violence, discrimination, neglect and abuse. Children's violence is divided into 2 basic groups, namely domestic violence and child school and extra-school bullying.
Violence in children aged 10 and over may eliminate the prevention of tobacco, alcohol and alcohol use; narcotics and psychotropic substances, computer addictions, slot machines. V adolescents are usually late in age and do not notice age. Also Childhood violence can eliminate education and training mental health, anger processing, child emotion processing, resolution of problems and crisis situations.
Commercial child sexual abuse is one of the worst forms of abuse children. It has untold consequences not only for the psychological and physical development of the child, but significantly dynamic a number of social and antisocial phenomena in the whole companies, literally with global consequences. We can plant it in the wider the context of the economic exploitation of children, of which other forms are e.g. the use of child labour, adoption of children for remuneration, use of children for committing crime, etc. It should also be understood as a specific type sexual abuse of children. Internationally valid definition of commercial sex child abuse was adopted at the World Congress against commercial child sexual abuse in Stockholm on 27 August 1996. Come on. "Commercial child sexual abuse' means the use of a child for sexual purposes in exchange for money or reward in kind between the child; and customers, intermediaries or agents and others who earn money from trade children for this purpose. Stockholm Congress also identified three forms commercial child sexual abuse:
- child prostitution " sexual exploitation for reward or the provision of other services,
- child trafficking - any transaction under which the child is sold by one person or group of persons to another person or group of persons for remuneration or other consideration,
- child pornography, any display of a child participating in actual or faked explicit sexual activity, whether it is display transmitted by any means and also any display the sexual organs of the child intended primarily for sexual purposes.
The Slovak Republic is inter alia bound by the Protocol to the Convention on Rights a child on the sale of children, child prostitution and child pornography in 2000, signed in 2001 and ratified in 2004, published in the ECR Law No 424/2004 Coll. recognising, inter alia, the importance of implementation the provisions of the Declaration and Action Programme against Commercial Sexual Abuse children admitted to the Stockholm Congress.
4. 1 Background
There is currently a lack of a single monitoring system based on analysis causes of the injury mechanism, severity, socio-economic status of the site, the financial difficulty of care, etc., more specifically drew attention to this the problem and the implementation of inter-ministerial cooperation has enabled efficient and preventive action in the area.
In Slovakia around 200 children and adolescents die every year as a result of transport accident, drowning, various falls, suffocation, burns and poisoning.
Mortality of injured and treated in trauma centers is 14% compared to by treatment in conventional hospitals, where mortality rates increase up to 45%. Child injury and mortality due to accidents clearly show inadequacy previous forms of prevention in our country.
Traffic injuries and domestic violence are particularly high. However, little is done for effective prevention as well as management and rehabilitation of consequences.
The Council of the Government of the Slovak Republic approved and proceeded with Infrastructure under the National Programme for the Safe Marking of Passways for pedestrians, which is aimed at the safety of passages near schools, safety school zones, railway crossings, etc. Road safety is chaired by the Minister of Transport, Post and Telecommunications of the Slovak Republic, is a permanent advisory and the coordination body and ensure the implementation of measures to reduce mortality in road traffic, including with regard to child injury. In particular, measures relating to the training of children, legislative measures, infrastructure and other preventive projects.
Basic measures include the implementation of an agreed concept for the development of children Transport courses, including the possibility to be activated with a view to increasing road safety in the second half of 2005 with a view to 2010. By year 2010 is planned to build about 55 - 60 children's transport courses. Main attention is paid to the revitalization of existing and post-transport areas the analysis is also expected to support the construction of new children's transport courses with to allow all primary school pupils to visit them in 2010 a nearby transport field.
As part of the implementation of the European Strategy discussed at the meeting of the Government of Slovakia 25. 4. 2007 is created a project of children's virtual transport field. Baby virtual transport field – With Martin on the road – is an animated computer game, in children aged 6-12 years are guided through individual traffic situations with which they are confronted daily. Illustrative form also offered critical situations in which they could engage in their activities find themselves if they do not comply with the basic road rules and offered prevention elements. This CD has the character of a teaching game with possibility to check knowledge learned. Children's virtual transport field With Martin on his way to the CD The Pedagogical Institute assessed by the Pedagogical Institute and approved by the MŠ on the basis of a recommendation Slovakia in October 2007 as a teaching aid for teaching transport education primary schools in Slovakia.
Expected legislative measures include the forthcoming proposal for a new Law on traffic on roads, driving conditions and change; and the addition of certain laws. in the collection of MV SR. Legislation must also be addressed Road safety issues with a focus on pedestrians, cyclists and vulnerable road users. It is primarily focused on visibility of horizontal and vertical traffic signs. The implementation of the project is left on the administrators of the PK - Class I roads - MDPT SR, class II and class III roads - higher territorial units and municipalities. This plan will be implemented on an ongoing basis. following years.
MDPT SR will be in the preparation of the new road law (now applicable law 135/1961 Coll.) remember safe zones and preventive construction and technical measures in terms of road safety.
Increasing road safety with a focus on pedestrians will be improved through General binding legislation on pedestrian safety in transport, restriction the speed in municipalities to the survival level of a collision with a motor vehicle, Preventive measures to prevent collision of pedestrians and motor vehicles the path of light devices or off-level communication (overpasses, Underpass) in places with high concentration of children.
In the field of prevention, preventive projects are being implemented in the Slovak Republic. In the whole Republic of the Republic of the Republic of the Republic of the Republic of the Republic of Latvia: V Other regional directorates are currently operating projects aimed at children's safety in road traffic - póla advises children On your way, you're not alone. "Safe in our city" buddy of mine, -Ride the skills of the bike, -The car is not an interpretation, bicycling, ready, watch out, start, car school for children.
In Slovak kindergartens, primary and secondary schools there are no complex Information, educational and methodological materials in the field of accident prevention. The materials appearing in schools in uncoordinated campaigns have short-term and partial scope. Children's education, but also parents' education in the area prevention of child injury in our country in the background. Prevention topics are taught fragmentedly, inter-subjectly, without context and context incompletely and comprehensively included in other subjects.

Realising the potential of physical activity for healthy mental development and personal development represents one of the key challenges and challenges of public health.
7. 1 Background
The Government Office of the Slovak Republic is preparing a medium-term strategy for the Roma national minority with by emphasis on socially disadvantaged groups for 2008 - 2015. Preparation The materials are co-ordinated by the Ministry of the Slovak Republic and the Office of the Government Commissioner for Roma communities and other practitioners.
In the context of compensatory measures, there is a need for the creation of "training" communication centres. Through the proposed training centres associated with health care and prevention will seek to capture and Support education and eliminate health problems in marginalized Roma communities, in individual communities linked to health care and prevention for young mothers and their children. In marginalized Roma communities often show early motherhood among underage women and from for this reason, many of them do not complete compulsory schooling and naturally do not continue to study in secondary schools.
The focus also applies to gender equality, because in segregated marginalized Roma communities are increasingly gender-based Disparities within their families and communities. In these centres they will be able to young mothers affected by early motherhood complete compulsory schooling attendance, continue to study in the selected curriculum and so get the skills that are now essential to the labour market. V to make greater use of the help and support of teachers, community social workers, community health workers; healthcare professionals in raising and caring for their children. For these children will also be so-called pre-school, but it will be more specific; because it will be extended to healthcare and prevention. Pre-school the facility will be accessible to all children without distinction. This medical care will be available to all Roma and Neromas living in the above mentioned micro-regions.
On the way to improving health care for children and adolescents marginalized Roma communities is strengthening the function of community healthcare professionals for a general practitioner for children; and Grow up. Along with a psychologist and social worker, he visits families, educating their mother about breastfeeding, and training practical skills in care for the child, hygiene and ensures vaccination of children and adolescents.
There is an increase in verbal and physical aggression in primary and secondary schools, bullying, violence, vandalism, use of narcotic drugs and psychotropic substances, schooling, torture and abuse, suicide attempts, pregnancy juvenile or delinquency and crime.
Problems also arise in the field of social relations in classes, communication with teachers, acceptance and tolerance of differences classmates, adequate conflict resolution, stress management, but also sound self-study, choice of appropriate learning style, professional decision making; and the election of the university and the field of study. These are problems that concern mental health of children and adolescents and their quality of life in school environments closely related to their success in study, school work ability and performance.
Problems in psychological services to schools can be competent and high professionally address the school psychologist as well as the teacher employee of the school; or school facilities.
Healthy psychosocial development and mental health of children and adolescents is not only the issue of marginalised Roma communities, but a healthy social climate create a majority community regardless of social status. It's high time create synergy solutions with a holistic view of health. Health care is a mental health issue addressed primarily at the level of psychiatric care without focusing on prevention; which is more efficient and disproportionately cheaper. Existing network pedagogical-psychological consulting facilities solve problems already encountered, most of which might not arise at all if knowledge about a healthy way of life that we have. Healthy start-up issues life is addressed within the first priority (health of the mother and newborn), however refers to the life of a child up to the age of max. 3 years. But the next period of childhood is no less significant and therefore it is important to address it with the same urgency as period of neonatal and period of adolescence. Pre-school age, school enrolment and the period of primary schooling involves a wide variety of family challenges; and represents a significant burden.
The impact of social factors on children's development has not been sufficiently addressed recently Attention, the transformation of society brought a lot of alarming negative impacts. These include, for example, a change in the founder of nursing institutions, a decrease in Health care in social care institutions, economics PLDD activities with reduced paediatric field surveillance (decrease in visitor services), there are no pediatric clinics, development is not detected volunteering and charity to cooperate and promote protection and consolidation the health of children. Among other things, the fact that Pediatrics have reached the edge of the interest of both professional and lay society.
Children's and Regrowing Periods of Life is important for shaping the foundations for healthy way of life in the area of proper diet, physical activity needs, meaningful leisure, personal development through life in family and wider community, i.e. outside school. Family with school children is is exposed to psychological, financial and social pressures, which are contained in at present very difficult and often leads to family crises and breakdown. Children are this a situation unnecessarily traumatized and such a model of a dysfunctional family They carry on transgenerationally. With a change in social conditions weakening external stabilisation factors, thereby increasing requirements for Personalities of parents and their readiness to ensure healthy development children in a healthy well-functioning family.
