The Ministry of Health of the Slovak Republic under the direction of Minister Kamil Šaška has systematically started to apply the legal institute of compensation difference pursuant to § 98i of Act No. 363/2011 Coll. Mechanism ensures that the holders of the registration of medicinal products after the expiry of the applicable period return to health insurance companies the difference between the real and conditional reimbursement for conditionally paid medicines.
–What has not been done for years has been implemented consistently. In one year, we decided to return more than 40 million euros and we prevented dozens of proceedings from being statuted. I've been working with my patient team since I got here. It's a practical expression of my promise to people. The law gives the Ministry clear tools to get funds back into the system. Any euro returned to health insurance companies can be used for healthcare for patients, stated the Minister for Health Kamil Šaško.
By decisions issued in 2025, the compensatory difference amounted to EUR 25 244 915.88 and thus to EUR 15 040 307,02 in 2026. In total, the Ministry decided to return more than EUR 40,2 million to the public health insurance scheme from summer 2025 to summer 2026
These results also show that the legal mechanism for countervailing differences has been applied inconsistently in previous years. Although the law required the Ministry to decide on the equalisation differences and to ensure the return of funds to health insurance companies, the previous management of the department did not systematically implement this agenda. As a result, the millions of euros that were to be returned to the public health insurance system for patient healthcare were not timely implemented.
These shortcomings were also highlighted by the Revision of the expenditure on medicinal products of the Value for Money Unit of February 2026, which recommended more rigorous use of financial risk sharing mechanisms and the active exercise of State claims. The SR Supreme Audit Office also identified serious shortcomings in the management of the system of classification of medicines and the protection of public finances in its control report of March 2026, the evaluation period from 2020 to 2024.

Minister Kamil After taking up his duties, he proceeded to immediate correction. Since its beginning, the resort has not only issued decisions worth more than 40 million euro, but has also saved about 30 proceedings, for which there was an imminent threat of limitation. The proceedings were opened just before the legal deadlines expired, thereby preserving the State's claims and protecting the funds for patients.
