International project Care4Diabetes (C4D) from 1 February 2023 until 31 January 2026 and was implemented in cooperation withNational Endocrinological and Diabetes Institute (NED). The aim of the project was to implement the pilot programme Let's reverse type 2 diabetes. to improve the health of type 2 diabetic patients through lifestyle changes.
Programme Let's reverse type 2 diabetes ongoing in two rounds. Each round consisted of intensive intervention phase maintenance after-care phases. Patient groups worked in hybrid format, which combined personal meetings and online activities. The program of each round started with the opening weekend meeting in NEDÚ.
The programme was implemented through of the multidisciplinary team, composed of programme coordinator, diabetic, psychologist, nurse and nutritional therapist, the team formed experts from the NEDÚ and provided management of the program in both rounds. Outpatient control visits and psychological consultations were conducted in Months 3, 6 and 12, where individual personal meetings with the multidisciplinary team were held on a monthly basis.
Participation in the programme 35 patients, where The age group 50'59 years was the most represented. During the intensive phase of the programme, mean weight loss 5.73 kg, to reduce the consumption of processed food and to increase the number of servings of vegetables in the diet. Following the implementation of the initial phase of the programme, patients also devoted on average 2.83 hours a week more motion and sport activities. Initial results indicate positive impact on all monitored parameters (weight, BMI, waist circumference, S-TK, D-TK, HbA1c, CHOL, HDL, LDL, TAG, visceral fat effect. At the same time, the programme The reverse type 2 diabetes was Very positive received and evaluated by patients themselves.
Project Care4Diabetes significantly contributed to the implementation of lifestyle changes in patients with type 2 diabetes and created solid basis for other structured patient health support programmes in Slovakia.
