Public invitation to apply for a subsidy from the Ministry Health of the Slovak Republic to support drug activities for the year 2021 (hereinafter referred to as "the call') shall be published pursuant to Article 5 (8) (a). (c) Law No. 525/2010 Z. zon the granting of subsidies under the Ministry of Health Slovak Republic, as amended.

1. Comprehensive legislation governing the granting of subsidies Ministry of Health of the Slovak Republic

2. Budget cleared for subsidies under the Ministry's chapter 2021; and an assumption for the next two years.

3. Procedure for applying for a subsidy

The invitation will be published by the Ministry of Health of the Slovak Republic (hereinafter referred to as "the Slovak Republic') On the website www.health.gov.sk (in the "Projects and Challenges" section of the project Drug policy side).

The call shall include:

• the date by which the application for a subsidy may be submitted,
• the method of submission of the subsidy application,
• the basic objective of the project,
• the application form in electronic form,
• available resources for the call,
• the highest and lowest amount of one subsidy,
• how to remedy formal deficiencies in applications
• the date of the evaluation procedure for applications,
• the composition of the commission,
• a draft subsidy contract.

4. The deadline by which a subsidy application may be submitted shall be until 19.9.2021.

5. Method of submitting a subsidy application

a)

The applicant submits complete documentation in documentary form, in Slovak the language, once the original of the application, signed by the statutory representative; the applicant, together with all the mandatory annexes, and one copy of the application without applicant identification, without mandatory annexes and once back-up on CD carriers or USB keys (scan or PDF format not allowed). Copy without identification shall be presented in a form that does not permit direct identification applicant (in application, description and structured budget not to be visible) the applicant's identification marks, address, signature and stamp). Original application including annexes as well as a copy of the application without annexes to be tied individually ridge binding so that parts cannot be inserted or removed documentation. ridge bonding will not be considered as cannons with by euro or other binding. The pages must be numbered.

b)

Application for subsidy together with the annexes prescribed by the applicant by post or in person. Time limit for submitting applications subsidies shall begin on the date of publication of the invitation to tender and end on the day indicated in the invitation to tender. to end the submission of applications. If the applicant lodges an application for a grant subsidies in respect of postal services, the date indicated on the postage stamps on the cover of the application. If the applicant submits a subsidy application personally, decides the date indicated on the stamp of the mailing office on the cover applications.
Envelopes or packages delivered and registered by the mailing office of the Ministry after the deadline will not be evaluated at all and will be returned on request to the organisation requesting the grant has been sent (the mail office of the Ministry receives the post in employment days at Monday – Thursday from 08.00-15.00 h, Friday from 08.00-14.00 h, ground floor, No. Door 1).

The application for a subsidy, including its annexes, must be lodged together in one sealed envelope which shall be clearly and visibly marked with the words:

Envelope content

1. application for a subsidy 1x original with identification of applicant (application, description the project, structured budget ) together with mandatory annexes,
2. application for a subsidy 1x copy without applicant identification (application, description the project, the structured budget) and without other mandatory annexes,
3. CD carrier or USB key (application, project description, structured budget with identification of the applicant, even without the applicant's identification).

c)

In the case of important notices from the applicant concerning changes in the applicant's the application submitted during the evaluation process can be communicated by email only: dotacieppa@health.gov.sk.

6. Basic objective of the project

The aim of granting subsidies is financial support for programmes, activities, innovative approaches and efforts to maintain the continuity of actions and initiatives undertaken in the drugs sector to cover drug prevention addictions, drug addiction treatment and drug resociation dependents.

Note: (Because there is no established epidemiological situation in Slovakia, COVID-19, MZ SR will not be Ministry in 2021 Support projects with the organisation of public events across the board (including actions in schools, cultural events, sports events, education), i.e. where high concentration of people is at risk in order to protect public health; Slovakia:

Priority areas for 2021:

1. Preventive programmes focusing on long-term and systematic work with children; and young people with drug issues.
2. Programs and activities focused on professionally guaranteed treatment, harm-reduction, and socialisation.
3. Purchase of material and technical equipment to carry out activities and activities in the drugs field.
4. Changes in construction or construction of equipment intended for prevention, treatment or the socialisation of drug addicts.
5. Professional meetings of drug workers (experts) fora and discussions on drugs).
6. Dissemination of drug education through mass communication technologies.

7. Application form in electronic form

8. Available resources for the call

Amount of funding allocated to grant support Drugrelated activities in the programme 0790203 - Health protection in the financial year 2021 is EUR 300 000,00.

9. Maximum and lowest amount of one subsidy

The highest amount is 50,000,00 EUR.
The lowest amount is 5,000,00 EUR.

10. Conditions for granting the subsidy

Subsidy under § 2 (1) (a) l) the subsidy law can be granted the applicant, who is a legal person or a natural person.

A subsidy may be granted to the applicant if it demonstrates that it has funding the project for which the subsidy is required to cover the costs of other resources of at least 5% of the total budget of the project.

The complete application for the subsidy submitted by the applicant consists of: mandatory particulars, namely:

I. subsidy application - published in Annex 2 to 4 to this invitation,
II. mandatory annexes (under the subsidy and budgetary law) Rules):

a) original or certified copy of the description of the project for which the subsidy is granted request, not more than three months old, on the date of submission of the application for a subsidy " (annex 5 to this call),
b) original or certified copy of the structured budget of the project, not more than three months on the date of submission of the application for a subsidy (annex no. 6 this call),
c) original or officially certified copy of the document identifying the statutory the applicant's authority, if the data contained therein cannot be obtained from the information general government systems according to a special regulation, not older than three months on the date of submission of the subsidy application,
d) evidence that the project is funded from other sources - own resources be certified by the original or certified copy of the declaration of honour that: the applicant shall provide 5% of the total project cost for project support and other sources are demonstrated e.g. by written commitment/commitment of a third party (other sponsor, city, municipality) to provide for a specific project and specific activity, the specific financial amount),
e) original of the applicant's declaration that the applicant has a financial settlement relations with the national budget,
f) the original or certified copy of the certificate of the competent bankruptcy court, that the applicant is not subject to insolvency proceedings; the restructuring and not rejected the application for bankruptcy due to lack of assets; not older than three months; this obligation is does not apply to an applicant who is an entity of the general government and a natural person - the competitor,
g) the original of the applicant's declaration that he is not against the applicant enforcement, this obligation does not apply to the applicant who is the subject general government,
h) original or certified copy of the certificate of employment inspectorate, not more than three months on the date of submission of the application for a subsidy that the applicant has not infringed the ban on illegal employment in the previous three years a special regulation,
i) data necessary to request a criminal record extract pursuant to Article 10 (4); letter a) or point (a) (b) the law on the criminal record, request the applicant's data. In case of consent, the form must be completed with the consent printed, signed and attached to the application by the applicant. In case of the applicant disagrees with the provision of data for obtaining a criminal record the natural person - the non-entrepreneur or the legal person this extract is necessary to request individual support for a subsidy application - (Annex) No 7),
j) confirmation that the applicant is registered in the Register of Public Sector Partners, for an applicant who has an obligation to register with partners public sector (Law No 315/2016 Coll. on the Register of Public Partners) sector and on amendments to certain laws) through an accessible public-sector partner register Ministry of Justice of the Slovak Republic.

On 1.12.2019 the amendment of Act No.177/2018 Coll. on some measures to reduce administrative burdens by using information public administration systems and amendments to certain laws (anti- bureaucracy) and also Law 523/2004 Coll. on the Financial Regulation of the Public reports and amendments to certain laws. From that date onwards, applicants they do not submit a settlement certificate as part of the grant application tax and financial relations, confirmation by the social insurance company that the applicant does not registered arrears for social insurance premiums and old-age allowances Pension saving and confirmation of each health insurance (Union Health) Insurance company a.s., Trust health insurance company a.s., and General Health Insurance company insurance undertaking, a.s.) that the applicant does not have registered underpayments of health insurance premiums insurance. The Provider shall obtain those certificates from the Information System www.oversi.gov.sk.

III. If the applicant applies for a subsidy for a change of structure or construction according to § 2 (2) 3 letter (g) the subsidy law annexed to the application is in addition to the above documents and:

a) details for the verification of the title certificate pursuant to § 60, paragraph 3 of the cadastral law The Ministry shall, on the basis of the applicant's data, request it. In case of consent, to be completed with the consent of the applicant to print, sign and attach the application. In case the applicant does not agree to provide data to obtain an extract from the title sheet of this extract must be requested individually; and to support the application submitted - (Annex No. 8),
b) a self-declaration from the applicant or a certified copy of the self-declaration the applicant who owns the property does not change the ownership right to property for at least 10 years from the date of the transfer of the building to use,
c) a self-declaration from the applicant or a certified copy of the self-declaration the applicant that the completed construction or the reconstructed construction will be used for the purpose for which the grant was granted, for at least 10 years from the date of construction completion or reconstruction,
d) written consent or certified copy of the written consent of all property owners with construction modifications, if the applicant is not the owner real estate,
e) final building permit or officially certified copy of final building permit or status notification document to the building office concerned or an officially certified copy of the declaration of construction document for the building concerned the office.

The above documents may not exceed three months on the date of submission applications for subsidies.

The subsidy shall not be granted to the applicant:

1. to cover commitments from previous financial years,
2. reimbursement of expenditure incurred in previous financial years,
3. who has been convicted of an offence against property or for another intentional offence,
4. for repayment of loans, loans and interest on loans and loans received,
5. to pay rent under the lease agreement for the purchase right of the rented item,
6. to equity in another legal person or to form or establish a legal person other than a budgetary organisation or a contribution organisations.

The eligible applicant may submit following the invitation and in the following the programme in the calendar year concerned only one subsidy application; in which it must not combine more unrelated activities.

11. How to remedy formal deficiencies in applications

Any application must be made in accordance with this invitation.
Requests made on a form other than that prescribed or modified, where appropriate, incomplete applications will be considered as temporarily non-compliant a formal assessment.
The applicant will be informed of this, the call for correction of deficiencies will be sent exclusively by e-mail to the e-mail address indicated by the applicant in the application where the applicant will be called upon to deliver the revised or completed a request within 5 calendar days of the date of receipt of the e-mail. Supplementary period the application starts on the first day following the date of receipt of the e-mail. If the end of the period shall be for Saturday, Sunday or holiday, the last day of the period; the next working day.
In case the applicant fails to deliver the revised application (s) within the time limit set, does not complete the missing documents in the application within the prescribed time limit, the application will not be evaluated and informed in writing.

Please note applicants that communication from the Ministry's tent regarding the correction of the deficiencies in the submitted application will take place exclusively by electronic e-mail and therefore make sure that you indicate in the application functional email address and regularly check content during the call an e-mail box.

The Ministry is authorised without consulting the applicant:

• to partially complete the missing applicant's identification data, if any annexed documents indisputable as the applicant,
• round the financial data in cents to the whole euro,
• correct the incorrect calculation of the mandatory minimum amount of own funds; or other sources of the applicant, in order to meet the challenge,
• adjust the amount of financial data reported in thousands of euros to euros,
• modify the wording of the purpose of the subsidy if it is clear from the request or justification what the applicant requests, or where the wording contained in the application could cause error,
• correct mathematical errors and complete missing totals based on numbers included in the budget of expenditure financed by the subsidy,
• modify the name of the activity/project.

Grounds for refusal of a request:

• an application for a subsidy was lodged after the date specified in that invitation,
• project activities do not meet the basic objective and priority areas for the year 2021,
• a subsidy application was submitted by an unauthorised entity (i.e. for activities, for which the applicant applies for funding does not have an organisation in the subject matter),
• the project, or part of it, will be implemented in other than the corresponding budget the year for which the subsidy is claimed,
• the applicant's self-declarations submitted to the application have proved to be false,
• the application for a subsidy has formal deficiencies beyond the deadline for their removal according to point 11 of this call.

12. Date of the evaluation procedure

The deadline for evaluating applications for grants shall be no later than: 10.11.2021.

13. Composition of the Commission

The selection and evaluation body is the Commission of the Slovak Ministry of Health Republics to evaluate the grant application established Ministry of Health of the Slovak Republic, whose composition and activities modifies the Statute published in the Journal of the Ministry of Health of the Slovak Republic the Republic of 29 June 2012 to the amount 17-19, which is annexed to this Decision Calls and Appendix No 1/2015 to the Statute of the Commission of the Ministry of Health Slovak Republic published in the Journal of the Ministry of Health Slovak Republic dated 19 June 2015, amount 4-5, which is annexed No. 1A of this call and Supplement No 2/2015 to the Statute of the Commission of the Ministry of Health Slovak Republic for evaluation of application for grant published in the Journal of the Ministry of Health of the Slovak Republic dated 13 August 2015 amount 8-9. The criteria for evaluating the grant application are: listed in Annex 1C to this call.

16. A model for the draft grant contract is published in Annex 9 to this Challenges

As a rule, the draft contract contains:

1. the identification of the parties,
2. the purpose and subject matter of the contract for which the subsidy is sought,
3. defining the time for which the contract is concluded,
4. the amount of the subsidy granted,
5. the conditions of use of the subsidy,
6. the current account number and name specifically established for the subsidy granted,
7. rights and obligations of the contracting parties (reimbursement period for unused financial resources the means and numbers of the accounts to which those funds are referred),
8. the conditions for the payment of the subsidy and the period for the levying of the proceeds from the subsidy,
9. penalties and penalties for breaches of financial discipline,
10. the reason and method of withdrawal.

18. Conditions for the payment of the subsidy

Pursuant to Act No 523/2004 Coll. on the Financial Regulation of the Public Administration and on amending certain laws as amended by § 8, paragraph 5 the law, the activities of the project must be carried out by 31 March 2022. For eligible expenditure may be treated only as incurred during implementation the project activities referred to in the approved budget.
