Public call for applications for a subsidy from the Ministry of Health of the Slovak Republic to support drug activities for 2019

Call for tender Ministry of Health of the Slovak Republic in support of drugs activities for 2019 (hereinafter referred to as "the call') shall be published pursuant to Article 5 (8) (a). (c) Act No 525/2010 Coll. on the granting of subsidies within the scope of the Ministry Health of the Slovak Republic, as amended.

1. Legislation governing the granting of subsidies competence of the Ministry of Health of the Slovak Republic

Law No 525/2010 Coll. on the granting of subsidies within the scope Ministry of Health of the Slovak Republic, as amended (hereinafter referred to as "the subsidy law') available at www.slov-lex.skk, in accordance with the National Drug Strategy of the Slovak Republic for the period Years 2013 - 2020.

Law No 523/2004 Coll. on the Financial Regulation of the Public Reports and amendments to certain laws as amended (hereinafter "the Financial Regulation Act') available at www.slov-lex.sk.

Act No 343/2015 Coll. on Public Procurement and on Change; and supplementing certain laws, as amended (hereinafter referred to as the "Law on Public procurements - available for www.slov-lex.sk.

Law 177/2018 Coll. on certain measures reducing administrative burden by using public information systems reports and amendments to certain laws available on www.slov-lex.sk.
Law No 330/2007 Coll. on criminal records and amendments certain laws (hereinafter referred to as the "Punishment Register Act") available on the www.slov-lex.sk.

3. Procedure for applying for a subsidy
The invitation will be published by the Ministry of Health of the Slovak Republic (hereinafter referred to as "the Slovak Republic') On the website www.health.gov.sk (in the "Projects and Challenges" section, Drug policy side).
The call shall include:
§ the date by which a subsidy application may be submitted,

§ the method of submitting a request for a subsidy,

§ the basic objective of the project,

• the application form in electronic form,

• available resources for the call,

• the highest and lowest amount of one subsidy,

• how to remedy formal deficiencies in applications,

• the date of the evaluation procedure for applications,

• the composition of the commission,

• a draft subsidy contract.

4. Date by which application may be submitted the grant is granted until 20 May 2019.

5. Method of submitting a subsidy application

a)
The applicant submits complete documentation in documentary form, in Slovak the language, once the original of the application, signed by the statutory representative; the applicant, together with all the mandatory annexes, and one copy of the application without applicant identification, without mandatory annexes and once back-up on CD carriers (scan or PDF format not allowed). The copy without identification shall be submit in a form that does not allow direct identification of the applicant (in the application, the description and structured budget must not be visible identifying features the applicant, address, signature and stamp). The original of the application, including the annexes, and a copy of the application without annexes shall be tied individually by ridge binding, in order to it is not possible to insert or select individual parts of the dossier. Behind the ridge bonds shall not be considered as "scannons' with Eurobalises or other bindings. The pages must be numbered.

b)
A subsidy application, together with the annexes prescribed, shall be submitted by the applicant by post or in person. Time limit for submission of applications for subsidies shall begin on the date of publication of the call and end on the day indicated in the call for closure of applications. If the applicant lodges an application for a grant subsidies by post, decides the date indicated on the postage stamp on the cover applications. If the applicant submits a subsidy application, he/she shall make a personal decision the date indicated on the stamp of the mail office on the application cover.
Envelopes or packages delivered and registered by the Ministry health care after the deadline will not be evaluated at all and will be returned upon request to the organisation which sent the subsidy application (subsidiary) Ministry receives mail on working days at Monday - Thursday from 08.00-15.00 h, Friday from 08.00-14.00 h, ground floor, No. door 1).

The aid application, including its annexes, must be delivered together in one sealed envelope, which shall be clearly and visibly indicated by the words:

Envelope content

1. application for a subsidy, 1x original with identification of the applicant (application, description, structured budget ) together with mandatory annexes,
2. application for a subsidy 1x copy without applicant identification (application, description, structured budget) and without other mandatory annexes,
3. CD carrier (application, project description, structured budget with identification the applicant, even without the applicant's identification).

c)
In the case of important notices from the applicant concerning changes in the applicant's the application submitted during the evaluation process can be communicated by email only: dotacieppa@health.gov.sk.

6. Basic objective of the project

The aim of granting subsidies is financial support for activities declared in the National Drug Strategy of the Slovak Republic for the period 2013-2020, support for programmes, activities, innovative approaches and also Efforts to maintain continuity in drug activities and initiatives areas to be covered by the prevention of drug addiction, the treatment of drug addictions and the resociation of drug addicts.

Priority areas for 2019:

1. Preventive programmes with a focus on long-term and systematic work with children and youth (peer activists, sports events, cultural events, competitions).

2. Programmes and activities aimed at professionally guaranteed treatment, harm-reduction, and socialization.

3. Professional meetings of drug workers issues (organisation of targeted events, expert forums and discussions on the drug problem).

4. Dissemination of drug education through mass communication technology.

7. Application form in electronic form

Model forms in electronic form for application:

1. (a) Application for a subsidy (non-entrepreneur) No 2 the challenge,

2. (b) Application for a subsidy (entrepreneur) (Annex) No 3 the challenge,

3. (c) Request for a subsidy (legal person) Annex 4 to this call,

4. (d) Description of the project: Annex 5 to this call,

5. (e) Structured application budget - Annex 6 of this challenges.

8. Available resources for the call

Amount of funds allocated for granting Support for drug activities in the programme 0790203 "Health protection" the financial year 2019 shall be EUR 633 000,00.

9. Maximum and lowest amount of one subsidy

The highest amount is 75,000,00 EUR.
The lowest amount is 5,000,00 EUR.

10. Conditions for granting the subsidy

Subsidy under § 2 (1) (a) l) the subsidy law may provide the applicant who is a legal or natural person.

The subsidy may be granted to the applicant if it demonstrates that he has Financing of the project for which the subsidy is required to cover the costs from other sources at least 5% of the total budget of the project.

Complete application for subsidy by the applicant it submits, consists of mandatory particulars, namely:

I. subsidy application - published in Annex 2 to 4 to this call,

II. mandatory annexes (under the Act on subsidies and the Financial Regulation Act):

1. description of the project for which the the subsidy is requested by þ (annex no. 5 of this call),
2. structured budget of the project (annex no. 6),
3. a certified copy of the applicant's establishment or establishment document, not older as three months on the date of submission of the application for a subsidy (e.g. statutes) organisation, organisation status, establishment list, foundation list, status the city, the founding contract, the founding document, the social contract, etc.), this obligation does not apply to the applicant declaring the establishment by law,
4. proof of the provision of financing of the project from other sources (own resources shall be demonstrate to the original self-declaration that the applicant provides project support 5% of total project costs and other resources shall be demonstrated e.g. written commitment/third party commitment - other sponsor, city, municipality, that provide for a specific project and specific activity from it, a specific financial amount),
5. the original of the applicant's solemn statement that he has settled financial relations with the national budget, not more than three months old on the date on which the request for subsidy,
6. original or certified copy of the certificate of the local administrator concerned taxes not more than three months old on the date of submission of the application for a subsidy, that the applicant does not have tax arrears; this obligation does not apply to the natural person, with the exception of a natural person, a businessman,
7. the original or certified copy of the certificate of the competent bankruptcy court, that the applicant is not subject to insolvency proceedings; the restructuring and not rejected the application for bankruptcy due to lack of assets; this obligation does not apply to an applicant who is public administration bodies and natural persons,
8. the original of the applicant's declaration that he is not against the applicant enforcement of the decision, not more than three months old, on the date on which the application for the subsidy, this obligation does not apply to the applicant who is a public body reports,
9. original or certified copy of the certificate of employment inspectorate, not more than three months on the date of submission of the application for a subsidy that the applicant has not infringed the ban on illegal employment in the previous three years a special regulation,
10. original or certified copy of the social security certificate, not more than three months on the date of submission of the application for a subsidy that the applicant does not have registered arrears for social insurance premiums and old-age allowances pension saving; this obligation does not apply to a natural person " the competitor,
11. the original or certified copy of the certificate of each health insurance undertaking, not more than three months on the date of submission of the application for a subsidy that the applicant has no registered underpayments of health insurance premiums (General Health) insurance company, a.s., Trust health insurance company, a.s., and UNION health insurance company, a.s.,) - certificates must be from all health insurance companies, regardless whether or not the applicant has employees, this obligation shall not apply to: the natural person ,
12. data necessary to request an extract from the criminal record criminal records § 10, par. 4, point. a) and points (b),
13. copy of proof that the applicant is registered in the register of public partners sector where the applicant is under an obligation to register Public Sector Partners (Act No 315/2016 Coll. on the Register of Partners) the public sector and amending certain laws),

III. If the applicant applies for a subsidy change of building or construction according to § 2 (3) (g) of the subsidy Act in addition to the above documents, the application shall be accompanied by:

1. data to verify the letter the name of the municipality and the cadastral territory, the title number, owner identification, parcel/parcel registry and parcel/parcel number, per whose plan is to build or inventory number of the building (according to cadastral law no 461/2009, § 60, par. 3),
2. a self-declaration from the applicant or a certified copy of the self-declaration the applicant who owns the property does not change the ownership right to property for at least 10 years from the date of the transfer of the building to use,
3. a self-declaration from the applicant or a certified copy of the self-declaration the applicant that the completed construction or the reconstructed construction will be used for the purpose for which the grant was granted, for at least 10 years from the date of construction completion or reconstruction,
4. written consent or certified copy of the written consent of all property owners with construction modifications, if the applicant is not exclusive property owners,
5. final building permit or officially certified copy of final building permit or status notification document to the building office concerned or an officially certified copy of the declaration of construction document for the building concerned the office.

The subsidy shall not be granted to the applicant:

1. to meet commitments of previous financial years,
2. reimbursement of expenditure incurred in previous financial years,
3. who has been convicted of an offence against property or for another intentional offence,
4. for repayment of loans, loans and interest on loans and loans received,
5. to cover own-account expenses, cover own-account losses third parties under the contract.

The eligible applicant may submit on the basis of the declared calls and in that programme, only one application for granting a subsidy and must not combine more unrelated activities.

11. How to remedy formal deficiencies in applications

Any application must be made in accordance with this invitation.
Requests made on a form other than that prescribed or modified, where appropriate, incomplete applications will be considered as temporarily non-compliant a formal assessment.
The applicant will be informed of this fact by e-mail and will be invited to Service of the completed application (s) 5 calendar days, from the date of delivery of the email. The period for completing the application shall begin on the first day following the day of delivery of the email. In case the applicant fails to deliver the revised application deadline or not to complete the missing documents in the application within the prescribed time limit, the application shall be withdrawn from the further evaluation on which be informed in writing.

The Ministry is authorised without consulting the applicant:

• fill in partially missing the applicant's identification details, if any of the other annexed documents the applicant in question,
• round the financial data in cents to the whole euro,
• adjust the amount of financial data reported in thousands of euros to euros,
• correct mathematical errors and complete missing totals based on numbers included in the budget of expenditure financed by the subsidy,
• modify the name of the activity/project.

Grounds for refusal of a request:

• application for a subsidy was submitted after the date of submission of the applications referred to in the invitation,
• project activities do not meet the core objective and priority areas for year 2019,
• a request for subsidy was made by an unauthorised entity,
• the project, or part of it, will be implemented in other than the corresponding budget the year for which the subsidy is claimed,
• the applicant's self-declarations submitted to the application have proved to be false,
• the grant application has formal deficiencies even after the expiry of the deadline for their removal according to point 11 of this call.

12. Date of the evaluation procedure

The deadline for evaluating applications for grants is no later than 30 june 2019.

13. Composition of the Commission

The selection and evaluation body is the Ministry's Commission Health of the Slovak Republic for evaluation of the application subsidies established by the Ministry of Health of the Slovak Republic composition and activity regulates Status published in the Ministry's Bulletin Health of the Slovak Republic dated 29 June 2012, amount 17-19, which Annex 1 to this Call and Appendix 1/2015 to the Statute of the Commission Ministry of Health of the Slovak Republic published in the Journal Ministry of Health of the Slovak Republic dated 19 June 2015, amount 4-5, Annex 1A to this call and Appendix 2/2015 to the Statute of the Commission Ministry of Health of the Slovak Republic for evaluation of application for grant published in the Journal of the Slovak Ministry of Health the Republic of 13 August 2015, to the amount of 8-9 set out in Annex 1B. The criteria for evaluating the grant application are set out in the Annex No 1C of this call.

14. List of applications approved and reason for approval

List of approved applications, including their date of approval, the purpose and amount of the subsidy requested and granted and the identification of the final the beneficiary will be published after their approval by the Minister for Health on the website of the Ministry of Health -Projects and Challenges Drug policy – Evaluation of the request for subsidies placed on the left parts of the website.

15. List of pending applications and reason for non-approval

List of pending applications, including the date and reason for them the non-approvals will be published after they have not been approved by the Minister for Health, Web site of the Ministry of Health, in the "Projects and Challenges' section Drug policy – Evaluation of the request for subsidies placed on the left parts of the website.

16. The model of the draft grant contract is published Annex 7 to this call

The model contract shall, as a general rule, contain:

1. the identification of the parties,
2. the purpose and subject matter of the contract for which the subsidy is sought,
3. the definition of the time for which the contract is concluded,
4. the amount of the subsidy granted,
5. the current account number and name specifically established for the subsidy granted,
6. rights and obligations of the beneficiary (reimbursement period of unused the funds and account numbers for which the funds are indicate),
7. the conditions for the payment of the subsidy and the period for the levying of the proceeds from the subsidy,
8. sanctions for breaches of financial discipline,
9. the reason and method of withdrawal.

Frequently asked questions will be published on the website Ministry of Health.

18. Conditions for the payment of the subsidy

Project activities must be carried out during the financial year 2019. The deadline for using the subsidy is 31 at the latest. 31 December 2019. Only expenditure which is eligible may be regarded as created during the implementation of project activities.
