Brief description of the project:
The main objective of the national project is to ensure and maintain personnel capacity in long-term and palliative care for persons with a long-term progressive disease. At the same time, there will be a lack of links between health and social care and coordination of health care and social services through support teams. These steps will strengthen their delivery at Community level. By providing social assistance, it will ensure that a person is released from the constitutional medical facility as soon as possible into its natural environment with social support. The support team may start its activities with a person placed on a so-called "acute' bed, thus helping to ensure the social background of the person in good time and prepare his family members for his or her home or other natural background. The support team shall serve as a tool to accelerate the transition from acute healthcare to subsequent healthcare and/or care in the natural environment of the person.
Basic objectives of the project:
The goal of the national project is to achieve the primary additional provision of personnel capacity, or increase the number of health and support personnel across the whole Slovak Republic, by creating support (multidisciplinary) teams, stabilization of professional staff in outpatient and institutional care providing long-term palliative care. The setting and implementation of palliative education will ensure the availability of care for patients in all regions of the Slovak Republic. By defining a support team in the law, it is important to set up a pilot setting for the introduction of these teams within regional hospitals, and subsequently to establish expert guidance. for their extension. Professional guidance/methodological materials will be the result of sub-activity 2 in the implementation of support teams in individual counties.
The objectives of the national project will be pursued through activities such as:
Main activity: Provision of additional personnel capacity in long-term and palliative care for patients with complex problems and/or dependent on assistance from another person
Subactivity 1: Stabilization of staff in palliative care and implementation of multidisciplinary approach in mobile hospices and hospices
