1. Who is the eligible applicant for the call 12I04-21-V12?
The eligible applicant is a healthcare provider according to § 4 (a). a), (b) and (c) of Act No. 578/2004 Coll. with permission issued pursuant to § 11 of Act No. 578/2004 Coll. on the operation of medical equipment with a professional focus: psychiatry, child psychiatry. Means of the mechanism to be providednewly born psychiatric stationer.
2. What is a mandatory part of the PPM application?
Applicant in the framework of the request for funding of the Mechanism (hereinafter referred to as the application for a PPM)must include additional annexes- a certified photocopy of the permit to operate a medical facility in accordance with
- Paragraph 11 (2) of Law No 578/2004 Z. z., as amended, where permissions for the operation of medical facilities are issued by the self-governing county in the context of the transferred performance of state administration,
- Article 11 (4) of Law No 578/2004 Z. z., as amended, where the permit is issued by MZ SR if more than one self-governing region is competent for the issue of the permit
- design documentation,
- valid building permit (if issued) or copies of the applications submitted necessary to obtain the necessary permits for implementation (according to the generally applicable regulations and Act No. 50/1976 Coll. - Act on Land Planning and Building Code - Building Act) (hereinafter referred to as "the Construction Act")
- statement of assessment and detailed budget of construction costs
- other mandatory annexes listed in the invitation
Authorisation to operate a medical facility shall be submitted by applicants who hereby demonstrate the provision of health care and, at the same time, the professional orientation which is decisive in the assessment of the applicant
3. What if the applicant does not have design documentation for the construction:
The applicant shall provide a description of the project and the budget of the project so as to be consistent with the other relevant documentation.
4. What if I lack an annex to be submitted together with the PPM application?
In accordance with Article 16 (3) of the Mechanism Act, if the application for the PPM is not complete, the implementing entity may request in the invitation/call for addition also to provide explanations and clarifications on the documents and documents relating to the application for the PPM.
5. How is it necessary to submit the original of the criminal record statement?
The applicant shall submit the original of the criminal record extract not older than 3 months sent in paper form in sealed envelope or by electronic means as a document with guaranteed conversion pursuant to a special regulation
6. When is VAT ineligible expenditure?
VAT is an unjustified expenditure (from the Applicant's Guide 7.b) if, the applicant is a VAT payer and has the right to deduct tax within the meaning of § 49 (2) of Act No 222/2004 Coll. on value added tax as amended
7. What is considered eligible expenditure?
A more detailed description of the eligibility of expenditure relating to eligible activities is in accordance with the measure MZ SR amending the yield of MZ SR of 10 September 2008 No 09812/2008-OL on the minimum requirements for staffing and material equipment of individual types of medical facilities as amended and Annex 8 to this call

8. Does the project documentation include information on energy savings?
Yes, the project documentation shall include information on energy savings, in accordance with the criteria. In both cases, the applicant shall demonstrate the achievement of this objective by means of an energy certificate of the building, which is the obligation for the builder under the provision of § 5 (2) (a). c) Act on EHB at the completion of a new building or significant renovation of an existing building - for the purpose of this ŽoPPM will submit a solemn declaration, which will be supported by real certificates at the end of the project implementation

10. Can the applicant submit and other annexes not mentioned in the call?
Yes, the applicant may submit other annexes related to the application for the LPA.
